to enter or change a patient's care team, complete the following steps click alter patient from the toolbar or click the patient tab then select alter patient the select patient screen appears where you can select the patient select the patient from the list and click ok you can also enter the patient's last name and first name in the top box and click search click ok the patient record appears click care team on the menu on the right click xxxx to add a new team the new care team screen appears click the team role drop down and select the role enter the provider responsible for the team click the provider type drop down and select between internal and external click the provider box and enter or search for the provider's name
