patient icon on the toolbar to open the “ select patient from provincial registry ” window patients can be searched for using one of the following criteria full name ( index field) full name & date of birth full name & postal code phone number hcn and assigning authority/province the maximum number of results returned is 50 if too many results are present, use more criteria to narrow your search press “ok” to open a new patient window with patient data populated the registered field will automatically default to yes click the apply button to save the patient if the data entered for the patient meets minimum business rule requirements, the patient will be saved and have last sent to provincial client registry updated press ok to close the window note if searching criteria does not meet the above search criteria, user click search button, message box display adding a new patient not existing in the client registry press the new patient icon on the toolbar to open the “ select patient from provincial registry ” window press the add local button to open a new patient window, without selecting an existing patient from the client registry enter the patient’s information as required by the business rules for the client registry if required information is missing, you will be prompted with a dialog notifying you of any missing information the “ keep list open” checkbox will allow you to leave this window open/to the side so that you may reference the list while entering the missing information, if necessary when finished entering patient information, click the apply button to save the patient if the data entered for the patient meets minimum business rule requirements, the patient will be saved and have last sent to provincial client registry updated to current date and time press ok to close the window note the phone fields require specific formatting the phone number must be entered as ten digits example 1234567890 if an extension is required to reach the patient, it can be entered by using an “x” to separate the extension from the phone number example 1234567890 x 123 entering the phone number in an incorrect format will produce the following error adding a new patient without sending to the client registry press the new patient icon on the toolbar to open the “ select patient from provincial registry ” window press the add local button to open a new patient window, without selecting an existing patient from the client registry enter the patient’s first and last names and date of birth leave the sex and hcn fields empty press apply an error window will appear to notify you of missing information required to register the patient in the client registry selecting yes will close the error window you should now see the registration status at the bottom of the alter patient window as last sent to provincial client registry never press ok to close the window patient register views along the left side of the patient register window, you will see a list of views each view of the patient register is used to enter or find a specific category of patient details, such as general contact details, date of birth, patient status, phn in this view, it is also possible to denote a patient’s preferred phone number and email address family next of kin, other family members, marital status personal alternative names, photograph the list of views that you see will depend upon your level of access and your organizational roles hot tip once the required fields have been filled in, you can click apply to save the new patient without closing the window several new views will then become available altering an existing patient record open an existing patient record by doing one of the following click in the toolbar, or select patient/alter patient from the menu bar the system will open the alter patient window for the patient whose name appears in the patient info panel this would happen, for example, if you have selected an appointment or patient record otherwise the select patient window will appear, allowing you to choose the patient see the user manual – introduction to profile for more information once you have modified the patient record (please see the sections below for more detail), and you would like to update the provincial registry, click the change master id button in the general view on the segment bar menu this opens the select patient from provincial registry window search for and select the patient whose record you would like to modify and click \[ ok] if the data entered for the patient meets minimum business rule requirements, last sent to provincial client registry will update to the current date and time click \[ ok] to save the information in the alter patient window and close the window selecting patient status open the alter patient window in the status field, select the appropriate status from the drop down list selecting patient label open the alter patient window in the label field, select the appropriate label from the drop down list a patient’s name will display in the selected colour in the appointment view and patient lists, providing a visual cue regarding certain patient details entering the provider and alternate provider open the alter patient window enter the name or click to search and make a selection the provider field is for the clinician to whom the patient is assigned or whom the patient wishes to see as a usual provider; the alt provider field is for the alternate clinician if their usual provider is unavailable next of kin (nok) open the alter patient window and select the family view enter the name of the appropriate person in the next of kin section if the person is already in the patient register, enter the name in the next of kin search field the other fields in the next of kin section will fill out automatically if the person is not in the patient register, enter as much information as you can into the nok name , nok address , and nok phone fields these are free text fields enter the person’s relationship , if known employer information enter data regarding the patient’s employer open the alter patient window select the social view enter the appropriate information in the search fields employer and occupation code fields if you would like to maintain a list of employers and occupations, or type applicable information in free text fields work name and occupation descr respectively patient notes and alerts notes area the notes view of the patient register allows you to add information regarding a patient using either free format text or a structured problems area open a patient record using alter patient select the notes view enter free format text information into the upper panel of this view the text formatting buttons are similar to those of microsoft word click when done problems area use the lower panel of notes view to record administrative problems related to a patient administrative problems are non clinical issues some examples might be “hard of hearing, so speak clearly,” or “has been referred to debt collector ” click to create a new problem record for the patient enter the relevant information into the problem administrative window that appears the only required fields are code which should be the word note, description a brief text description of the problem if you would like this problem to trigger an alert message, follow the instructions in the next section when done, click \[ ok] turning a problem into an alert ordinary problems are simply recorded in the patient’s record, and are seen only when someone selects the notes view sometimes, however, problems should trigger alerts for example, if a patient is a bad credit risk, this fact should display during billing operations if the patient is a known drug seeker, an alert should pop up when making an appointment or opening a new encounter to prescribe medications follow these steps to turn a problem into an alert while entering data into the problem window, check the alert box below the alert checkbox is a list of system events, such as booking or altering an appointment or opening the patient’s medical record place a check next to each event that should trigger this alert hot tip if a problem has already been entered, you can reopen it by clicking on it in the notes view and then clicking icon care team the care team view specifies the providers contributing to the patient's care some providers will be implicit in their involvement, because they have done an encounter, or have otherwise performed some action for the patient others will be explicitly chosen to access the care team , click on the view in the patient register a new team role can be added by clicking on the icon this opens the new care team window date added the date the team member was added this defaults to today's date but can be changed if required added by this read only field denotes who created this record team role select the required option from the drop down list options in this field are short codes of type care team roles provider the provider responsible for the team role this field consists of two parts a drop down list to select the type of provider ( internal or external ), a selection box for the provider type in the name or click to search through a list of suppliers or database of internal or external providers note some team roles, such as usual provider , can only be internal status the status of the team member ( open or closed ), depending on whether the member is actively involved in the patient's care principal tick this box if the specified provider is to be the principal care team member for the team role specified above note a care team may have several principals, but only one active member for each team role may be designated as the principal comments any relevant comments can be entered as free text in this field case if relevant, specify the case to which this care team role applies privacy specify the privacy role for this care team member, if required date closed the date the member stopped care closed by this read only field is populated when the status of the care team member changes to closed , and denotes who closed it click close to close out this member from the care team a 'closed' team member simply means that the member is not actively involved in the patient's care the status can be changed back to open later, if required click \[ok] to close the new care team window alternatively, because the team roles are defaulted in the care team view, a specific provider can be entered by clicking in the provider field next to the specific team role and typing in the name of the internal or external provider the type can be changed if the team role is an external role appointments window load criteria the load criteria allow you to determine which appointment information you want to display you can specify the pos (place of service), prov (provider), and date calendar the calendar displays the current month, allowing you to navigate by clicking on any day in order to view the appointment load for that day you can view a different month by selecting the or arrows, or by clicking on the month name or the year and selecting a different month and/or year from the drop‑down list hot tip the toolbar button will return the calendar to the today view waiting room the waiting room area provides a quick view of those patients who have arrived and are currently waiting to be seen, with a time clock kept of their waiting time the patient arrival will automatically show in the provider’s work centre “being seen” area the being seen area lists those patients who have been shown to an examining room it identifies which room they are in and which provider they are seeing patients who are presently with the provider are shown with the icon, while those still waiting are marked the patient holder the patient holder enables you to temporarily store a patient’s name and appointment details, as a reminder that some action needs to be completed in relation to that patient a useful feature of the patient holder is that the patient can then be dragged to an appointment slot, and all patient details automatically recorded toolbar icons for appointments display several toolbar icons control the display of appointment information also called attendance load you can displays appointments for one day, one week, or one month displays appointments for a group of providers at a time;for example, the doctors group, or the nurses group, or all providers this is called the multi‑provider view show or hide the navigation bar area making a standard booking the book appointment window from the appointments view of the work centre , open the book appointment window using either of these methods double click on an empty appointment slot, or right click on the appointment slot and select book from the drop down menu the book appointment window will appear selecting the patient type the name of the patient into the patient field use the same format ( surname, first name ) as with the select patient window partial name searches are allowed; a list of matching names (if any) will appear in a drop down list double click the desired patient name, or click new to enter the name of a new patient you can also click the ellipsis icon to bring up the standard select patient window selecting the appointment type click on the current appointment type, then the ellipsis icon and select the desired appointment type from the list, or start typing the name of the appointment type and select from the dropdown list the appointment will take on the time and colour parameters associated with that appointment type additional topics for more information, please reference the following topic sheets work centre – group sessions opening an appointment for editing double click the appointment cell; or select the appointment cell and click in the toolbar; or right click the appointment cell and select edit from the pop up menu this brings up the edit appointment window, and allows you to modify the same fields as book appointment with the exception of the patient’s name extending an appointment to extend the duration of a patient appointment, either select the appointment cell, click its bottom border, and drag it to the desired duration; or open the appointment for editing (as above), and select a new duration in the for field changing the provider to change the provider responsible for an appointment open the appointment for editing (as above), select the with field, and enter the code of the new provider note the with field is a search field for the provider codes in the database deleting an appointment a booking can be deleted if it has been entered in error select the appointment cell and either click in the toolbar, or right click the cell and select delete from the pop up menu cancelling an appointment when a patient calls and cancels an appointment, it should be recorded as such and not simply deleted select the appointment cell and either click in the toolbar, or right click the cell and select cancelled from the pop up menu the cancelled appointment is still visible (for reference) on the appointment view, but an empty appointment slot opens up just to the right of it hot tip there is a preference setting that allows you to hide cancelled appointments from the display to set it, go to organisation/preferences/appointments on the menu bar double booking to book two appointments for the same provider and the same time slot right click the appointment cell and select double book from the pop up menu the two appointments will appear side by side on the appointment view moving an appointment select the appointment cell, drag it to the desired target cell, and drop it as you drag, a yellow tag will follow the mouse pointer, indicating the start time and duration of each target cell a confirmation box will appear click yes to move the appointment hot tip if the target cell is not on the same display as the original appointment cell, drag the appointment to the patient holder field then search for the new appointment, and drag the patient there from the patient holder field a confirmation window will appear select the appropriate button ( move, new or cancel ) to proceed with the move if the appointment type or duration of the destination slot is different from the original appointment, an action box will appear asking which values to use if you want to retain the duration and appointment type of the original appointment, uncheck these boxes if you wish the new appointment to take on the duration and appointment type of the destination slot, leave these boxes checked copying an appointment if you wish to copy an appointment to a new location, but leave the original appointment unchanged, perform the following steps select the appointment cell hold down the \[ctrl] key drag the appointment to the desired location (or to the patient holder ) and drop it there release the \[ctrl] key in other regards, the procedure is the same as for moving an appointment searching for appointment slots profile allows you to quickly search for booked, cancelled or free appointment slots go to work centre/appointments view click on the work centre toolbar to show the quick search panel in the date from field, enter the start date in the date to field, select the time range from the dropdown list in the time from/to fields, enter the time range or time of day from the dropdown list in the last field in the pos/prov fields, enter the pos and provider, if necessary in the type field, enter the type of appointment, if necessary select your action select the booked radio button if you are searching for booked appointments and enter the patient’s name in the for patient field, or select the cancelled radio button if you are searching for cancelled appointments and enter the patient’s name in the for patient field, or select the free slots radio button if you are searching for free appointment slots click the search results will show in the right hand panel hot tip if you selected to search for free slots, you can book an appointment by double clicking in the desired slot in the search results panel to open the book appointment window click to close the quick search panel hot tip the quick search panel can also be accessed from special/appointment search handling patient appointments updating patient status “arrived” you can “arrive” a patient in any of a number of ways click on the appointment, and while holding the left mouse button down, drag the appointment to the waiting area click on the appointment, and then on the arrive icon right click on the appointment and select arrive from the pop up menu a patient who has been marked as “arrived” will appear in the waiting area with an hourglass icon the wait column will measure how long the patient has been waiting note wait time can be measured from arrival time or from the start time of the appointment moving a patient to an exam room you can indicate that the patient is now waiting in an examination room click on the patient in the waiting area and drag to the appropriate exam room in the location column right click on patient’s name in the waiting area , select assign location from the drop‑down menu and select the room the patient’s name will be displayed in both waiting and exam room areas until the patient’s record is activated updating patient status “seen” when the patient is seen by a clinician the patient’s status will change to “seen” when the provider starts entering clinical notes for the patient the patient disappears from the waiting area, and the icon in the exam room area changes from an hourglass to an eye updating patient status “close attendance” upon completion of the patient visit, the attendance must be closed by right clicking on the appointment (in the attendance location or the appointment slot), and selecting close attendance from the pop‑up menu this step can be completed by either the provider or the administration staff the patient disappears from the exam room the appointment slot is marked to indicate that the attendance is closed reviewing attendance times profile records the patient’s arrival time and the beginning and end of the attendance according to the actions described above booking an unalloted appointments the appointments view shows the hours of availability of each provider, and the standard length of appointments for that provider to book an appointment outside of normal hours, do one of the following click the new appointment icon in the toolbar; or position the mouse pointer at the beginning of the desired time; click and, holding down the left mouse button, drag to form a box of the desired size hot tip a yellow tag will follow the pointer, giving a running indication of the start and end times of the appointment slot in either case, the new appointment window will appear enter the required information into the fields, paying special attention that the time panel correctly reflects the start time , the duration ( for ), and the provider ( with ) booking a walk in appointment if a patient arrives without an appointment, book a walk in appointment select the provider for whom you want to book the appointment by clicking on the provider’s name at the column header click the walk in icon in the toolbar; or right click anywhere in the provider’s schedule, and select walk in from the pop up menu the walk in booking window will appear this is similar to the new appointment window, but with fewer fields since there are no time details the appointment time will be registered as the actual time of booking, and the provider who is currently highlighted on your window will be assigned the appointment enter the patient’s name in the patient field click \[ok] the patient will appear immediately in the waiting area blocking out the schedule blocking out an entire day you can block out an entire day for a provider who is not taking appointments select the day in question in the appointments view right click the provider’s name at the column header, or the day in question if there is only one provider select block out from the pop up menu in the confirmation window that appears, fill in a reason for the block out if you wish, and click \[ ok] blocking out part of a day you can block out parts of a provider’s appointment schedule click the block out icon on the toolbar the mouse cursor will turn into using the block out cursor, click at the beginning of the time to block out, and drag out a box covering the period to block out release the mouse button at the end of the desired block out period hot tip a yellow help tag will follow the cursor, summarising the blockout period in the confirmation window that appears, fill in a reason for the block out if necessary click \[ ok] the blocked period will appear greyed out on the appointment schedule printing the appointment list printing a full day sheet open the appointments view for the day in question click the print list icon in the confirmation window click all to print the appointments for all displayed providers; or click selected to print only the appointments for the highlighted provider printing the appointment list for selected appointments only to print appointments for a time range, follow the above steps, but in the confirmation window, select the appointments after/appointments before radio button, and fill in the desired times printing labels go to work centre/appointments view select an appointment click in the toolbar this opens the appointment labels box select the all labels radio button if you would like to print out all labels for the day select labels after/labels before radio button if you would like to print out labels for selected time ranges click all to print labels for all displayed appointments click selected to print labels for selected appointments only the select a label box will appear select a label, and the number to print click \[ ok] note if you would like to print out a label for a patient that is not displayed on the appointment window, click on the label icon on the main toolbar of profile unmatched transactions most lab results and other incoming transactions are matched automatically to the appropriate patient and provider, but if some of the information used for matching is incorrect or different, they will have to be matched manually viewing unmatched transactions go to control centre/manage/manual matching in the main menu manual matching hot tip only the items in italics are unmatched; i e , if the patient name is italicised, the item needs to be matched to a patient; if the provider name is italicised the item needs to be matched to a provider manually matching transaction to a patient click an item in the list of ‘ unmatched transactions’ on the left side of the window possible matches for the patient appear in the best guess patients panel choose one of the following options to match the transaction drag and drop the transaction on to a patient’s name select a transaction, then select a patient’s name and click assign select a transaction, and then click find to search for a patient once matched, the file is removed from this view and moved to the appropriate patient record and the doctor’s unsigned transactions view manually matching transaction to a provider choose one of the following options drag and drop the transaction on to a provider’s name in the lower right corner select a transaction, then select a provider and click assign select a transaction, and then click in the other field to search for a provider not in the list if the transaction has come to your organization in error, select it and click delete once matched, the file is removed from this view and moved to the appropriate patient record and the doctor’s unsigned transactions view scanning documents the scan processor allows you to scan images and documents and import them into the system, linked to a patient’s medical record to scan a document load the document you want to scan into the scanner select special/scan processor on the menu bar the scan processor window appears in the look in field, click to browse for and select the folder to store the scans when finished, the scanned document is listed in the scan processor as “unknown” with the scanning date previewing documents to preview a document that has already been scanned click on the desired scan from the list on the left hand side of the window the preview appears on the right hand side use the toolbar to manipulate the image delete the scan move the scan up or down in the list refresh the current view zoom in click the button, and then click on the document to magnify it zoom out click the button, and then click on the document to reduce the magnification rotate the image 90 degrees select a portion of the document, and then crop it grouping documents to group multiple pages of a document together hold the \[ctrl] key and click on the documents, and then click linking documents to link scans or groups of scans to a patient and provider select the scan or group of scans you want to link to a patient and enter the required information in the fields to copy the document(s) to a provider, click to open the select provider to copy to window in the window, click to add internal providers this opens the select people and places window select the provider you wish to whom you wish to copy the document and click \[ ok] click click apply hot tip click the checkbox to ensure the scan will be signed off with the name of the provider to whom it has been linked file locations and categories you can select the destination of a scan using the file location dropdown menu the dropdown menu contains 3 listings which have their own categories documents letter referral consents advance directives admission and discharges email predict transcription complaints cardio respiratory event summary information disclosure scanned documents results pathology radiology encounters set retain data properties it is useful to turn this feature on when scanning files in batches and each scan needs to retain one or more properties for the current session to retain fields click on retain the set retain data properties window comes up, listing all the fields whose data can be remembered while processing the scans tick as required and click ok clicking on on/off toggles between the two options when on is selected, the retained fields will be highlighted in a turquoise colour select a scan and process as usual on clicking apply , the selected data fields will remember and carry over the data entered in the previous scan onto the next scan the next scan will be ready to be processed toggle the setting back to off when no longer required importing documents once scans have been linked to a patient and provider they can be imported into the patient’s medical record and the unsigned transaction view to import scans click to start the import process all scans in the current directory that have been linked to a patient and provider will be imported click \[ ok] on the confirmation dialog depending on the number of documents being imported, this may take a few minutes when finished, the scans are removed from the scan processor window hot tip all imported scans are also removed from the folder where they were scanned if you wish to save a copy of the documents on your disk drive, copy them to another folder before importing the scans can now be viewed in the patient’s medical record the category selected when linking the scan to a patient determines what view of the medical record the scan will be viewed viewing documents in the medical record documents pertaining to a patient can be viewed in the medical record the documents are correspondence related to the patient like scanned documents or letters go to medical record/documents view click the document to be viewed in the list of documents in the document preview panel below the document can be viewed to view the document in actual size to maximize image view you can also double click on the document in the list of documents to open the document in a separate window hot tip you can use the find fields to search for documents select from the dropdown list to find the documents by the description, the keywords, or the body in the next field, enter the text click button to search moving a document to a different medical record if the document is in the wrong patient’s medical record, it can be moved to the correct patient’s medical record click this will open a dialog box asking if you want to move the document click yes this will open the select patient for move window select the patient click \[ok] the document is moved with annotation stating from which patient medical record the document was moved entering measures to add measures, follow these procedures open the clinical details window for a patient select the measures tab by default, three key measures height, weight, and blood pressure are available in the transaction panel different items can be added in the source field, select embedded observations double click the desired observation from the available items panel in the transaction panel, if required, select the date and time for the observation if nothing is entered, it will default to the current time and date click in the value field and enter the measurement double click in the column to indicate that the value is abnormal entering problems to add coded problems for the patient, follow this procedure open the clinical details window for a patient select the problems tab click the add problem window opens note if you click the down arrow you can select to add a problem of a particular type diagnosis , procedure , or social/risk enter a code in one of the following ways type the icd 10 number type part of the name a list of possible matches is shown double click the required code from the list enter any other required details click \[ ok] to save this problem outward referrals reviewing outward referrals the referrals view in the work centre shows a list of the referrals meeting the criteria specified in the selected filter to open the referrals view select the referrals view of the work centre window hot tip filters allow you to specify the criteria for which referrals to view; for example, to view referrals based on the date they were sent, or referrals that require action the providers panel allows you to see to whom the referral has been sent and who is sending the referral the tracking panel shows you the progress of communication to the patient regarding appointment information the appointment panel allows you to change the status of the referral and will show you appointment information for the referral patient when the referral process is complete; change the status to closed this will remove the referral from the pending referral list if you require more information on the original referral select the button managing outward referrals as the referral moves through the system you will need to update various details, including the progress of the referral from the progress drop down list the external appointment date/time and any advice to give to the patient about the appointment checking the patient informed box automatically updates the progress field when the referral process is complete, choose closed from the status drop down list interventions you can use interventions to manage and track intended follow up visits for patients these may be single visits or part of a series such as annual physicals or cervical smears viewing a patient’s interventions to view the interventions for a patient go to clinical/care plans in the main menu, or from the patient’s medical record, click care plans in the segment bar \ screenshots\ care plans jpg using the intervention list run the intervention list on a regular basis to generate a list of patients to be seen in the future use this list as a basis for contacting patients and arranging scheduled interventions to open the intervention list, do one of the following in the clinical segment of the work centre , select intervention list , or go to report/care plans
