viewing patient modified accession items accession settings docid\ vfi1gp ii 5hbsv85e85h accession patient portal patient accession allows the patient to be an active participant in their care depending on the access permissions set up by the practice, the patient can make appointments, request prescription refills and review their clinical information, among many other actions this document describes all areas of the patient accession web portal logging in to the patient portal when accession is licensed, your clinic will be given a url to provider your patients with to use the accession web portal access to the web portal is granted on a patient by patient basis, and is configured within profile more information on granting permission to the web portal can be found in the accession configuration documentation when the patient navigates to the url, they will be prompted to enter their username and password after successful login, they will be in the web portal at the home view home the home view shows headline information, configured by the practiceusing the headline manager found in organization/headline manager , and any created alerts for patients the alerts are configured as mentioned later in the document, under accession patient configuration appointments the appointments view shows past and pending appointments to book an appointment, select a date range in the look between fields, select the appropriate pos, provider, and appointment type, and click search from the available appointments, click book to request the appointment appointments booked through accession appear in real time in profile in the edit appointment window audit tab, the log will show that the appointment was booked by the patient an additional notice appears in the provider’s work centre/accession items view please refer to the accession items section of this document on viewing patient modified accession items appointments will only be available for booking through accession when the associatedappointment rule in maintain/appointment/rules has “permit accession” checked with the associated accession profile problem list the problem list view shows the current problems for the patient, including diagnoses, adverse problems and procedures select ? at the end of each problem to access a summary and web link to more information by default these are blank and must be customized by the practice these are configured by going to organisation/preferences/accession/accession patient/text tab select the problem list/info option this opens a new window, where a list of coded problems can be created for which you wish to add additional information care plans the care plans view shows interventions that are still due or all interventions, depending on which radio button is selected appointments for the interventions can be made from here using the make appointment button these appointments will automatically be linked to the associated intervention in profile prescriptions the prescriptions view shows current prescriptions and allows the patient to ask for prescription refills, pending approval by the provider this is the one interaction that does not appear in the work centre/accession items view the information will show directly in the work centre/prescription refills view the prescription refills module may not be enabled for your practice further documentation on prescription refills is available on the intrahealth self service centre website investigations the investigations view is similar to the results view in profile, lab results and radiology results can be seen from here referrals the referrals view is similar to the referrals view in profile current and past inward or outward referrals can be seen from here note hovering over the external provider provides a hint box showing contact information from the external providers list in profile flow sheets the flow sheets view is similar to the flow sheets view in profile chronic disease observations from existing cdm forms are tabulated for comparison clinical outcomes the clinical outcomes view is similar to the measures view in profile measures are displayed based on date range and can be displayed as either a graph or table, using the show as dropdown clinical notes the clinical notes view is similar to encounters view in profile clinical notes are shown and can be sorted by their associated problems reference series the reference series view is similar to the new encounter/reference series view in profile for patients with existing reference series/growth chart data, the appropriate reference series can be selected from the drop down menu and displayed the show values checkbox will enable labelling the measure values directly onto the graph linked clinical records the linked clinical records view allows patients to add links to other patients’ accession portals, such as their family members to link other accession enabled patients press the add link button to add another patient enter the user name and password for that patient’s accession portal the patient will now be visible in the my linked clinical records section to open another patient’s accession portal, simply click on their name patients who have had their clinical record linked through another patient’s accession portal will see the names of patients who they have been linked with in the my record is linked by section new clinical note the new clinical note view allows a patient to enter his or her own clinical notes and/or observations patient created clinical notes appear in the work centre/accession items view to be reviewed by the provider the provider can sign the note, which will then appear as an encounter in the patient’s medical record, with the author being the patient there are two panels in this view the top clinical note panel is a free text area where notes can be typed the bottom clinical observations panel can be used utilized for custom forms these are the same cdo forms as found in or created in maintain/templates/ health record collections they are linked to accession in the organisation/ preferences/accession/accession patient/templates tab more information on templates can be found in documentation for accession configuration note as with other forms in profile, creation is an involved process and advanced training is required ask a question the ask a question view allows the patient to ask the provider a question patients can attach files to their questions, such as picture questions appear in work centre/accession items view, to be answered by the physician account the account view is similar to display transactions in profile financial transaction history for the patient is shown here if turned on however, these permissions can be customized by the provider to dictate how much access the patient will have to his or her records, and to the practice information double click on the default profile in the accession profiles table to access the permissions left click in the value field for the property and enter the modifications each category has both a write property and an enable property as an example, patients can be allowed to see their appointments but not create new ones if the enable value is yes but the write value is no a description of the preference selected is visible in the bottom panel note additional accession profiles can be created by left clicking back in the accession profiles table toolbar so that different patients can be granted more or less access than others text the text tab allows for text to be added for the different views or events in accession patient, wherever there is more information , information , or a help button to edit any question options to be chosen, or a password macro to be used the require acceptance of terms and conditions checkbox will require patients to accept the terms and conditions as defined in the custom texts tab upon first login templates the templates tab allows forms to be used in accession, either provided by intrahealth or created by your practice left click to show forms left click the category accession profile click ok to save other the other tab contains short codes for question categories question categories are populated in the ask a question view of the patient portal these can later on be used in the accession items view to filter through patient questions, so that they may be directed to the appropriate party managing accession related fees accession patient allows for multiple optional revenue sources the following explains how these fees can be maintained and tracked by a clinic under organization / preferences / accession / accession patient , in the setup tab, double click an access profile (in this example, we are using “default”) under the “ fees ” property, multiple service codes are listed accessionmed default service code for billing of medication refill requests this fee will be automatically billed to the patient upon an approved medication refill from accession accessionmon default service code for billing of accession monthly fee this fee will be additionally, if a patient asks a question through the patient portal ask a question view, a fee can be assigned when answering the question in the accession items view doing so will automatically create a private invoice to the patient for the amount specified the financial provider can be specified for the invoice the invoice as generated from the above screen accession items patient accession allows the patient to be an active participant in their care depending on the access permissions set up by the practice, the patient can make appointments, request prescription refills, etc this document describes how to view and accept or decline items created or modified by patients in profile viewing patient modified accession items the work centre/accession items view shows activities and items that have been performed in accession this can include appointment requests, patient created clinical notes, changes to demographics, and questions appointments – make any changes management purposes to load a list of patients based on criteria, the following fields function as below group will, as above, load a specific group of patients grouped previously in this view profile refers to the access level profiles as set up accession patient preferences these can be accessed and edited at organization / preferences / accession / accession patient , under the setup tab enabled
