# Article Overview & Feature Requirements

This article covers how Administrators or Clinicians can sign discharge medications in the **Discharge Reconciliation&#xA0;**&#x76;iew and load them into the **Usual Scripts&#xA0;**&#x76;iew. Since the patient is leaving the facility, future prescriptions are prescribed from the **Usual Scripts&#xA0;**&#x76;iew rather than from the **MedChart** view.

This action is performed by a user with prescribing authority who can confirm that:

- The list of discharge medications is correct AND
- The list should be uploaded under **Usual Scripts** in the patient's Medical Record, replacing the existing usual scripts.

## Feature Requirements

- Profile version 10.0.8 or higher
- MedChart should be enabled by adding the Medications licensed module.
- Role granted: **Sign Discharge Medication/Load to Usual Scripts**

- The medications have been [loaded from the MedChart view into the Discharge Reconciliations view](docId\:WeTqGnlcPXVu8uzxYKVoi) and any needed [edits to the list of medications](docId:5yuqihYLbKiRPv3m_OwUq) have been completed.

# Sign Discharge Medications and Load to Usual Scripts

## Steps

To sign medications in the **Discharge Reconciliation** view and load them to the Usual Scripts view:

1. Open up the Medical Record for the applicable patient.
2. Select **MedChart&#xA0;**&#x6F;n the left-hand side. The MedChart view is selected by default. 
3. Select the **Discharge Reconciliation** view.
4. Click on **Sign and Upload**.(This option is visible *only if* the **Sign Discharge Medication/Load to Usual Scripts&#xA0;**&#x72;ole has been [granted](docId\:YYC5Bqk-UoD9HujA_SSQI).)

5. You will be asked to confirm whether you want to authorize and sign the discharge medications, and upload them to the Usual Scripts list, where they will replace the existing usual scripts.If you are ready to do so, click **Sign**.

## Results

Clicking **Sign&#xA0;**&#x68;as the following results:

- Authorization and signing of all discharge medications.
- The user performing the signing action is recorded in the **Signed By** column.

- The scripts in **Usual Scripts** (accessible at **Prescriptions/Usual Scripts** and in **New Encounters/Scripts/Usual Scripts**) are replaced with the list of medications in the **Discharge Reconciliation** view.

- The scripts previously in **Usual Scripts&#xA0;**&#x61;re now considered discontinued and can be seen in the Medical Record under **Prescriptions/Discontinued**. The comment "Replaced with Discharge scripts" indicates the reason for discontinuing the scripts.

Now that the Discharge Medications are in **Usual Scripts**, they can be prescribed in the usual way during an outpatient Encounter.

**Note:&#x20;**&#x41;fter clicking **Sign**, the lists of medications in the **Discharge Reconciliation&#x20;**&#x76;iew and the **MedChart&#x20;**&#x76;iew remain uncleared.
