## Profile's Admission Medication Reconciliation Feature

| Application Change(s) | Admission Medication Reconciliation is a critical process in healthcare aimed at ensuring that patients' medication information is accurately and completely documented when they are admitted into an inpatient care home.
Profile introduces a new Medication Reconciliation feature designed to provide a seamless workflow for managing patients' medication information during inpatient care admissions.
The key components of this new feature include:
* **Comprehensive Medication List:** Collecting and documenting a complete and accurate list of all medications the patient is currently taking, including prescription medications, over-the-counter drugs, herbal supplements, and vitamins.
* **Verification:** Ensuring the collected information is accurate and complete.
* **Two-Signature Workflow:** Introducing an optional two-signature process for reviewing and approving the medication list.
* Conversion to Prescriptions: Converting the appropriate medications into signed prescriptions ready for administration. |

## Prescription Subsidy Code Change

This is now covered through implementation of a new service component of '2'.
Where a customer has the Scripts and Instructions\General\Prescription Subsidy preference set to **Unapproved Prescriber** and a patient who holds a CSC card is receiving a prescription, the subsidy code applied to the prescription shall be for example **A2** for an Adult, **S2** for a Senior, **J2** for a Junior and **Y2** for a Youth. |

| Chat Time Display Changed to a 24-Hour Format in French Environments |
The time display in the Chat window for a French environment is now presented in the 24-hour format. The time display in the Chat window of an English environment is presented in the 12-hour format. |
| Improved Form Saving Experience with Status Bar Notification | We've streamlined the form-saving process to enhance user experience. Previously, a pop-up window appeared when saving a form, but this has now been removed. The status of saving the form as a PDF is now conveniently displayed in the status bar of the main window. This update aims to provide a smoother and more efficient workflow by keeping you informed of progress without disrupting your task. |
| Appointment Audit Enhancement | An enhancement has been made to the appointment audit. When a patient has declined an appointment via an SMS reminder, the audit will now show the appointment was cancelled/declined by the patient, and a default user \ will be used for the entry. Previously the Provider was listed, which had caused some confusion, as providers generally do not confirm/cancel appointments. |
| Managing Duplicate Patient Records | When Profile automatically identifies duplicates and presents the dialog to manage the duplicate record(s), users have the following options:
1) ‘**Mark as not duplicate’** - When selected, this will perform the same action as the current ‘Remove and Ignore’ action.
- This assumes that these are not considered duplicates.
- “Ignored in Find duplicates search” in the Alter Patient record will be set so that these records do not appear in find duplicate search window.
2) ‘**Ignore and Save’ **- Will keep the duplicate. This action assumes they are still duplicate and next time will be prompted again
- Records will still be included in duplicate search
3) ‘**Cancel’ **- no action has been taken. Dialogue closes
4) '**Show' **- Same action as current show option |

External references that previously caused significant delays (20–30 seconds) and timeouts during email rendering are now handled more efficiently, preventing timeouts and ensuring smoother processing. |
| Encounters | - The "End Encounter" UI design has had some minor improvements made to it, bringing it more in line with the overall style found throughout other areas in Profile.
- After completing a form that had not been printed, the linked action status in the encounter was automatically set to Actioned. This caused an issue during the End of Encounter process when the form had not been printed, so this step was skipped. This has been fixed and the action status will no longer be set to Actioned. |
| Financial | * When printing a transaction or invoice to A5 paper, the invoice was printed on two pages and some information was cut off. This has been fixed.
* In cases where a Provider has multiple Payee IDs setup for a POS and when a user selects one the Payee ID in the billing window, the same should reflect in the billing list view. There was a bug where there was a discrepancy between the Payee ID selected in the billing window and what was displayed in the billing list view. This has been resolved.
* When an appointment was made for a Provider who has the 'Create Claims: On Appointment' preference set, the claim date was incorrectly set to the date it was created. This has been fixed and the claim date is now correctly set to the date of the appointment.
* In the Find Accounts\Edit Filter, no overdue accounts were being displayed when users were setting the “For Period” to 3 months, despite there being accounts that have financial information relevant to this filter setting. This has been fixed.

| Forms | - Not all interface components were translated fully or correctly in the new Profile Marketplace integration. This has been fixed.
- When opening a form that has multiple tabs, these tabs were hidden until you hovered the mouse over them. This has been fixed.
- Fixed an issue where Forms were not importing from the new Profile Marketplace integration into the French Profile environment.
- Fixed an issue where Document and Typing Templates were not importing from the Profile Marketplace integration into the French Profile Environment. |
| hCare | * There was an issue where changes were not reflecting immediately in an encounter that had been updated. |
| Medications | - The Authority Number for Special Authority forms are populated using a special calculation which includes a check-digit. This calculation would not work when the base number exceeded 9,999,999, and prescriptions were being rejected as invalid by pharmacies. This has been fixed.
- When a previous prescription order was sent via email, the Supplier field was disabled for any new orders created. This has been fixed, and any previous orders will be ignored during the email process so users can select the Pharmacy to send the prescription order to. |
| Notifications | * An exception error was seen when loading Unsigned Transactions Work Notifications if the filter was set to filter by a Provider Group which was not available at the current POS due to Organizational Filtering and the option was selected in the Provider Group to “Limit to this level and below”, referring to the POS or Org Node the Provider Group was owned by. This has been fixed. |
| Obstetrics | - A practitioner should be able to state the patient is a non smoker and still be able to state that the patient has been exposed to second hand smoke. This option was greyed out unless they choose "Yes" to Tobacco. This issue has now been addressed. |
| Patient Management | * Resolved an issue in the "Find Patient" feature where searches using the first digits of a mobile number (including the leading "0") would return no results unless an asterisk (\*) was added. |
| Patient Record | - When viewing a patient's cases, encounter entries are not displayed in the correct chronological order. This has been fixed.
- An issue occurred when patient names were lost after booking an appointment via online booking. The description on their appointment showed their name, but their name was removed on their chart. This has been fixed. |
| Portals | * An issue has been fixed in Profile Online Booking where the rebranding colour changes were not applied on some elements. The listed elements differed from the design colours.
* There was a translation issue with the Telemedicine feature. When the voice/video devices were unavailable the messages were not fully translated to French.
* An issue occurred where appointments booked using Online Booking were changing the Mode from ‘Phone’ to ‘In Person’. This has been fixed. |
| Referral | - There was an issue where Medicare claims for referrals were being rejected due to an incorrect date being given. |
