# Overview

This article is meant for clinical and non clinical users. It covers the functionality of ordering lab requisitions, and CDO forms in Profile to showcase the **Show as PDF&#xA0;**&#x69;con available in a patient's **Medical Record&#xA0;**&#x61;nd in the **Work Centre**. This article also provides some background around where the medical record data (included in the PDF Export functionality) is generated from.

# Order and View a Lab Requisition Form in PDF Format

An ordered lab requisition form can be viewed in PDF format. 

## Order a Lab Requisition Form 

When a provider opens a lab requisition form in the **New Encounter&#xA0;**&#x66;ield by first selecting the appropriate form from the form drop-down list, and then selecting the **Order form&#xA0;**&#x69;con in the **Pathology&#xA0;**&#x6F;r **Radiology&#xA0;**&#x74;oolbar, a form with the patient's details gets generated.

The provider can then fill out the remaining items in the form.

When they are done, the provider has the option to either select the **Save** or the **Save and Print&#xA0;**&#x69;con. Doing so takes a snapshot of the completed lab requisition form, which is then stored in the patient's medical record data.

**Note:&#xA0;**&#x54;his is the process that makes the **Medical Records&#x20;**&#x50;DF Export functionality available. A patient's entire medical record can be exported and viewed as a PDF. This means that any lab requisition forms, CDO forms, prescriptions, encounters etc. can be exported and viewed as a PDF. This can be useful if a patient requests access to their medical record, and/or if a provider needs to share a patient's medical record with another provider. **Note:&#x20;**&#x41;ny form that is saved is included in the PDF Export of the patient. There is no way to exclude any forms. It is not possible to print the form in a different format later on either, as the PDF presentation will be captured once the form is saved.

## View a lab requisition Form in PDF Format 

If the provider would like to view their completed lab requisition form in pdf format, they can do so by selecting the **Show as PDF** icon.

The **Show as PDF&#xA0;**&#x69;con can be found in the following places:

- **Medical Record/Encounters/Forms**
- **Medical Record/Forms**
- **Work Centre/Forms**

# Order and View a CDO Form in Profile

It is also possible to view an ordered CDO form in PDF format. 

## Order a CDO Form 

A provider can find/open an appropriate CDO form either in the **Medical Record**/**New Encounter/Action Panel/Forms** field by selecting **Browse&#xA0;**&#x6F;r in the **Most Used Forms&#xA0;**&#x70;anel.

Once the form has been selected, the form generates and the provider can fill out any necessary information. 

When they are done, the provider should **Save** the form. Doing so takes a snapshot of the completed lab requisition form, and it is then stored in the patient's medical record data.

The provider then has the option to **Print** or **Email** the form using the appropriate icons in the toolbar.

## View a CDO Form in PDF Format
