# Overview & Requirements

Profile integrates with Ocean MD's Website Forms for patient referral and new patient intake. This article provides guidance for administrators who are configuring and managing Ocean MD's Website Forms as well as managing patient medical records in Profile.

## Requirements

- Some of the functionality described in this article requires Profile 10.0.1 or later.

# Configuration and Administration

The workflow to configure and administer patient referral and intake into Profile using Ocean MD's Website Forms is as follows:

- Enable Importing Website Form Submissions from Ocean into Profile
- Create and Configure Website Forms
- Send Email Link to Patient
- Accept Patient Submission
- Result

## Enable Importing Website Form Submissions from Ocean into Profile

In order for Profile to be able to create a new patient record using the form data from Ocean, the Administrator needs to do the following in the Ocean interface:

1. Go to **Menu/Admin**.

2. In the **Admin Settings**, under **Additional Functionality**, click on **Site Features**.
3. Ensure that **Allow accepted website forms to create patients in your EMR** is checked.

4. Click the **Menu **button and then click on **eForms**.
5. Find the Ocean eForm that you are using for your Website Form, and open the eForm in the** eForm Editor**.
6. Ensure that your eForm has a mandatory item that asks for the patient's health number and that the **Populate answer for EMR field** field for that item is set to "hn" (in the **Note Formatting** tab).
7. Ensure also that your form has mandatory fields that are mapped to corresponding EMR fields for the patient's first name and last name, in addition to their sex.

## Create and Configure Website Forms

Next, create a new Website Form Link in the Ocean interface:

1. Go to **Menu/Admin**.

2. On the **Admin Settings **page, under **Additional Functionality**, click on **Website Form Links**.

3. On the **Website Form Links **page, click the **New Website Form Link Configuration** button in the bottom right corner.
4. Next, configure your Website Form Settings, and set your Website Form Notification Email.For details on how to do this, consult the [Create & Configure Your Website Forms](https://support.cognisantmd.com/hc/en-us/articles/115003448311-Create-Configure-Your-Website-Forms) guidance from Ocean.

## Send Email Link to Patient

Next, copy and paste this link into an email and send it to the patient. 

The patient then clicks on this link, fills out the form, and submits it to Ocean.

## Accept Patient Submission

Finally, to trigger the creation of a new Medical record in Profile using this submission as imported data:

1. In the Ocean platform, go to **Menu/Website Forms/New**.
2. Open the submission you want to import and, if the patient form submission is satisfactory, click on **Accept** to trigger the import into Profile and categorize the submission into the **Accepted** folder in Ocean.

## Result

Once you have accepted the patient submission, Ocean looks for an existing patient in Profile with the matching health number. If one is not found, a new **Medical Record **is created in Profile, with the demographic information in the submitted form. 

The submitted form is also automatically imported into Profile as a PDF Document, accessible in the **Medical Record/Documents** view.
