The COVID-19 Immunisation Register (CIR) is to be decommissioned in 2023 and the National immunisation Register (NIR) will be replaced with the Aotearoa Immunisation Register (AIR). AIR will replace the CIR and NIR as the master system for recording immunisation event data.  

These changes are due to occur on the 2nd-3rd of December, 2023 with a go-live date of Monday 4th December. All messages to the NIR will be paused at 0100 on Saturday the 2nd of December to accommodate this change and will be processed when the new system is enabled at 0900 Monday 4th December.

Customers who administer immunisations will be required to upgrade to the latest 8.8.12.E version of Profile by the 4th of December.

As a consequence of this and other requirements, the following changes will be included in the Profile 8.8.12 release:

- Deactivation of the ability to launch CIR from the main toolbar button
- Removal of Opt-off options
- Enabling the sending of COVID-19 immunisations from Profile
- New COVID-19 care plan (3 primary and 3 booster doses)
- New COVID-19 vaccine codes
- New Indication codes to support privately funded immunisations
- Updates to the childhood immunisation care plan to include serology reports

Further changes will be applied by March 2024:

- MenB recall schedule update
- HPV9 recall schedule update
- Providing the ability to send all immunisation events to AIR for all NZ administered vaccines
- Providing the ability to send all immunisation events for patients whom had previously opted-off

Follow the instructions below to apply all changes to Profile. 

# Configuration Steps

## Importing jaffa files for the AIR release package

Import all jaffa files contained in the Profile AIR Immunisation Changes - December 2023.zip release package by extracting all the files from the folder, then navigate to **Organisation\Import & Export\Import Jaffa Folder...** in Profile.

The package will contain jaffa files for: 

- COVID-19 care plan (replaces the current 3-dose with a 6-dose care plan)
- Service codes for all COVID-19 immunisations and any new privately funded immunisations
- Vaccine codes for all NZ administered immunisations
- New indication codes for privately funded Primary and Booster doses
- IMMAdhoc care plan to record any non-scheduled immunisation notifications
- Childhood Immunisation (CI) Care Plan with Serology Report at 5 and 8 months
- LAUNCHCIR - Redirect CIR to care plan macro

## Removing the CIR button from the main toolbar

The CIR (COVID Immunisation Register) is where NZL vaccinators recorded COVID-19 immunisations.  This will no longer be the case and vaccinators are required to record these in Profile.  In a previous release to customers, Intrahealth provided the CIR main toolbar button and LAUNCHCIR macro so users could access the CIR from their main toolbar.  Users will be required to remove this toolbar button manually using the 'Add or Remove buttons' function in the main toolbar. 

Downloading and importing the macro LAUNCHCIR - Redirect CIR to Careplan.jfa distributed will force any user that hasn’t removed the toolbar button to open EMR\Care Plans when the CIR toolbar button is clicked. A notification will be displayed to the user as a reminder.

# Summary of Changes

## COVID-19 immunisation care plan

The new COVID-19 care plan contains a total of 6 doses.  The default vaccine applied to the primary doses is the mRNA-CV 30 mcg vaccine and the default vaccine applied to the booster doses is mRNA-CV 15/15 mcg vaccine.  These primary and booster doses can be swapped with the administered vaccine by using the Alternative workflow below:

1. Complete the intervention by either double-clicking the intervention/immunisation action or right-click the action and select Complete.

2. In the intervention action tab e.g., COVID19-30MG, click the

3. Select the vaccine and click **OK**.

4. Select **Yes** to replace the current action.
5. Enter the details of the vaccine e.g., Route, Site, Batch, Expiry and Sequence and complete it by clicking **OK**.

## Serology Report added to the CI care plan

Previously, Hepatitis B serology reports could be logged directly with the NIR system.  This is not the case for the AIR and will require a new release of the CI (CI2020B) care plan which includes serology report interventions at 5 and 8 months of age. Users can select from 4 possible outcomes accepted by AIR:

1. Protected (NIR-P)
2. Indeterminate (NIR-IND)
3. Unprotected (NIR-NTP)
4. Parent declined (NIR-PDS)

## Sending a Serology Report

In order to send a serology report for the patient, the patient must have a previous childhood immunisation with a completed status.  Follow the steps below to send a serology report:

1. Complete the HepB 5- or 8-month intervention Result action in the CI2020B care plan.

2. Select the Outcome for the Result action.
3. Set the Status to **Complete** if it has not been set to Complete.
4. Click **OK** to save the intervention.

On completing the intervention, Profile will resend a previous immunisation message as an update and include the serology report in the message.  See the example below:

## Notes

- Importing the new COVID-19 plan will not overwrite patients whom have a current COVID-19 care plan in their record.
- COVID-19 immunisations should be recorded in Profile (similar to all other immunisations) and the immunisation event message will be sent on completion to AIR.
- References to NIR have not changed and are still visible in Profile.  These may change to AIR in the future but are not required at this time.
- Patients can no longer opt-off sending immunisation event details.  These options have been removed from Profile.
- Previously there were rules applied based on the NIR go-live date or patients date of birth which restricted immunisation update messages from being generated and sent. These rules have been removed and all immunisation completions and updates will be sent to AIR.
- Vaccinators can also complete COVID-19 immunisations using the Immunize Now function in EMR\Immunisations.
- All overseas COVID-19 vaccines should be recorded under a single vaccine code 500 and the service code COVID19-OS.
- Invoices and claims are not generated when completing COVID-19 immunisations, as the payments are handled by the AIR system.
