# Overview & Requirements

This article is intended for all EMR users. It will demonstrate how to add, configure, use and delete Forms in Profile EMR. 

## Requirements

- An active account for Intrahealth Online Knowledgebase: [https://help.intrahealth.com/en/support/solutions](https://help.intrahealth.com/en/support/solutions)
- Profile Role Actions Granted
  - Organization\Import and Export\Import Jaffa File
  - Organization\Import and Export\Import Jaffa Folder
  - Maintain\Templates\Forms
  - Organization\External Providers

**Note**: Any changes you make will affect all users inside your EMR container, including other locations and physicians that you share your EMR container with.

# Add, Configure and Delete Forms in Profile

The steps below can be used by all users to add new or updated forms, or remove forms no longer being used in Profile.

## Step 1: Download a Form

Go to [Intrahealth Knowledge base](https://help.intrahealth.com/en/support/solutions) and login with your email address and password.If you forgot your password, click **Forgot Password**.If you do not have an account, contact our support desk via email (help\@intrahealth.com) or by phone (855-841-7387) to request an account.

In the Search box **Find some solutions here...&#xA0;**&#x74;ype one of the following (Or a combination)

1. Form ID
2. Form Title

Click the result to see a preview of the form accompanied with the form files to download. Alternatively, click **View All** to see all form results.

Click each attachment to download each file to your computer. If the attachment is a ZIP file, please download the ZIP file and unzip the contents. 

1. For a single JFA form, proceed to Step 2: Import a Form.
2. For multiple JFA forms, create a "New Folder" and move all JFA files to the new folder.

## Step 2: Import a Form 

Open Profile EMR to import a form.

**Note:&#xA0;**&#x43;aution - Importing a form that already exists under the same name will *permanently* overwrite it. Overwritten or deleted ***customized&#x20;***&#x66;orms are not recoverable. Original Copies can be re-downloaded from the [Knowledge base](https://help.intrahealth.com/en/support/solutions).

Click Organization / Import & Export / \<Choose A. or B.>

1. Import Jaffa File for single attachments.
   1. Browse for the single JFA file you downloaded in Step 1.3 above.
   2. Select the JFA file and click **Open**.
   3. Click **Start** in the JAFFA file Import window.
   4. The import finishes when the top heading displays "**Jaffa Import Completed Successfully**". This process is usually between 0-30 seconds or longer for large forms.
   5. The import log shows the directory the form is stored

2. Import Jaffa Folder for multiple attachments.
   1. Browse for the "New Folder" you moved the form files into in Step 1.3.a above.
   2. Select the JFA File and click **OK**.
   3. In the JAFFA file Import window, Click **Start**.
   4. The import finishes when the top heading displays "**Jaffa Import Completed Successfully**". This process is usually between 0-30 seconds; longer for large forms.

## Step 3: Configure a Form for Use

Use these instructions to configure an imported form.

### Non-requisition Forms

1. No additional configuration required.

### Requisition Forms

1. Go to **Organization** / **External Providers**
2. Set the **Type** drop-down list to **Suppliers** and the **Specialty** drop-down list to **Radiology** or **Bloodwork** (**Pathology**)These fields must match the form you are importing.
3. Locate the form you recently imported. Double-click to open the **Edit External Provider** window.
4. Click the **Order Format** view.
5. Click the **Format** drop-down menu and select **Scanned Form**. You should now see a visual preview of your form.
6. Click **OK** to save the form.

## Step 4: Use a Form with a New Encounter

Use these instructions to use a form in a New Encounter clinical workflow.

### Non-requisition Forms 

1. From the **Medical Record Encounters&#xA0;**&#x77;indow or Main Toolbar, select **New Encounter**.
2. Select **Form** in the **Actions** pane.
3. In the **Look For** box, type the title of the form and click the **Find** button.
4. Double-click the form to open or select the form and click **Selected**. Alternatively, you can click the **Browse** button to browse all forms, then select the form and click **OK**.
5. Fill the form before printing or faxing.

### Requisition Forms 

1. From the **Medical Record Encounters** window or Main Toolbar, select **New Encounter**.
2. Select **Radiology** or **Bloodwork (Pathology)** in the **Actions&#xA0;**&#x70;ane.
3. In the middle pane, click the drop-down to select the desired form.
4. Add investigations and click **Order Form** to view / edit the form before printing or faxing.

## Step 5 (Optional): Delete a Form

Use these instructions to delete requisition and non-requisition forms in Profile.

### Delete non-requisition forms

1. Go to menu Maintain / Templates / Forms
2. Locate the form in the correct folder.
3. Right-click the form, click Delete.
4. Click Yes to confirm.

### Delete Requisition Forms

1. Go to menu **Organisation / External Providers**
2. Set the **Type&#xA0;**&#x74;o **Suppliers** and **Specialty&#xA0;**&#x74;&#x6F;**&#xA0;Radiology** or **Bloodwork&#xA0;**(**Pathology**)
3. Click **Search&#xA0;**&#x62;utton.
4. Select the form, click **Delete External Provider&#xA0;**&#x62;utton on the toolbar.
5. Click **Yes&#xA0;**&#x74;o delete the form.
