--- title: Generating a Hearth Health Plan slug: portal-sub/generating-a-hearth-health-plan docTags: createdAt: 2026-03-04T12:03:22.643Z --- # Cardiovascular Disease Risk Assessment Calculator: Generating a Heart Health Plan This functionality allows clinicians to collaborate with the patient, using a guided process, to identify patient goals and determine realistic steps that can help manage the patient’s cardiovascular risk. The process culminates in a personalised Heart Health Plan resource (generated by Health NZ) that patients can take home, summarising their cardiovascular health assessment and providing clear, practical actions the patient can take to improve outcomes. ## Requirements - This functionality is available from Profile 10.0.11 onwards. - This feature is only available in New Zealand, as it relies on integration with Health NZ's CVDRA API. - The user must have a Provider Number applied to their record. - The eMessage Service must be installed and the API Key entered for the eMessage service. This API key is given to Intrahealth by Health NZ. Speak to your Intrahealth Customer Service Representative to arrange for this to be configured for you. - Users must first perform a [CVD Risk Assessment](docId:6HPRr9bMJU_mdtWKrtn5m) before generating a Heart Health Plan. Note: A CVD Risk Assessment calculation is a pre-requisite to obtain a Heart Health Plan. Enter the number of **Months to next visit**. This is a mandatory field. 2. Discuss goals related to CVD risk with your patient and enter these goals (up to a maximum of 3) into the dialog under **My goals are**. 3. Discuss steps to reach those goals with your patient and enter these steps (up to a maximum of 3) into the dialog under **Steps I will take**. 4. Choose between 1 and 3 relevant **Interventions **from the list provided. Click **Submit**.
