--- title: Flow Sheets (8.6+) slug: portal-sub/flow-sheets-86 docTags: createdAt: 2026-03-04T12:03:22.636Z --- # Flow Sheets Flow sheets allow the monitoring of patient data over time, by collecting and displaying data from CDM forms, lab results and manually entered measures. The patient data is displayed in two tables, based on a specific CDM form, with the first table being a list of CDM form items and their latest values, and the second table being a cumulative view of the values in dated columns.  Prescription information and immunisations/vaccines can also be displayed, and new transactions can be added. The following flow sheets are available in Profile: - Asthma - Congestive Heart Failure - COPD - Diabetes - Hypertension Flow sheets can also have variants, which are provider-based and/or patient-based customized flow sheets. These can have different items, target ranges and time intervals than the core flow sheets. ## Requirements To use flow sheets and create reference ranges and intervals, the following role actions must be granted: - **Clinical\Flow Sheets** - **Clinical\Flowsheets\Set Reference Range** - **Clinical\Flowsheets\Set Variant Intervals** To add or remove information in flow sheets and maintain settings, the following role action must be granted: - **Maintain\Flow Sheets** To create flow sheet variants, the following role action must be granted: - **Maintain\Flow Sheets\Create flowsheet variants** For New Brunswick, the role actions are granted automatically as part of the upgrade. ## Workflow 1. Select the desired flow sheet in the **Flow Sheets** panel on the left side. The measures and lab results are displayed in the **Items** column; the most recent data is displayed in the **Last** column, and the date the data was entered or captured is displayed in the **Date** column. Once you have finished entering the values, press **OK** to save the information and close the window. This information will now display in the flow sheet. ### Target Ranges Target ranges can be specified for flow sheet items based on the value ranges that are pertinent to the patient. 1. In the **FlowSheet** field, enter or select the flow sheet to which this concept target range applies if it should be different for the patient’s other flow sheets. 4. In the **Provider** field, enter your provider code if this target range is only to be used by you. 5. In the **Provider Group** field, enter the provider group code or name if this target range is only to be used by a specific provider group. 6. In the **FlowSheet** field, enter or select the flow sheet to which this concept interval applies if it should be different for the patient’s other flow sheets. 4. In the **Provider** field, enter your provider code if this interval is only to be used by you. 5. In the **Provider Group** field, enter the provider group code or name if this interval is only to be used by a specific provider group. 6. If the latest value of an item exceeds the interval, the item and the latest value in the Items table will also display with a yellow background. ## Creation of Flow Sheet Variants 1. In the **Name** field, edit the text to describe the variant. This could be as simple as adding the provider’s name or patient’s name or it could be adding meaningful text or an abbreviation describing the difference of this particular CDM flow sheet. 3.
