Go to **Medical Record/Immunisations** 2. When complete, press **OK** to save the Immunisation to the patient’s Medical Record. ## Historical Entry using Immunise Now The Immunise Now feature may be used when there are single ad-hoc immunisation records that need to be entered. However, a user-defined date and status in the 'Immunise now' window does not get registered in the patient's Medical Record.  To rectify this, the user must double click the newly added record and, in the subsequent **Care Visit Completion** window, re-enter the actual completed date in the **Set Date Concluded to** field, and select **Complete** in the **Status** field. Upon saving this information, the correct data will be reflected in the patient's medical record. For more information about the Immunise Now feature, see [Immunise Now](https://help.intrahealth.com/en/support/solutions/articles/6000237302). ## Immunisation using Care Plans A Care Plan is a structure for multiple interventions which may include or be totally comprised of immunisations. For example, childhood immunizations, which are given over years according to regional schedules. Using a Care Plan lets you know when to remind patients to come in to receive immunisations. Completing an immunisation by creating a Care Plan means the immunisation being given will be filled in automatically depending on the Care Plan used.
