To enter or change a patient's Care Team, complete the following steps:

Click **Alter Patient **from the toolbar or click the **Patient** tab then select **Alter Patient**.

- The Select Patient screen appears where you can select the patient.

Select the patient from the list and click **OK**.

- You can also enter the patient's last name and first name in the top box and click **Search**.

Click **OK**. 

- The patient record appears.

Click** Care Team** on the menu on the right.

Click **XXXX** to add a new team.

- The New Care Team screen appears.

Click the **Team Role **drop-down and select the role.

Enter the Provider responsible for the team:

- Click the **Provider type **drop-down and select between **Internal** and **External**.
- Click the **Provider box** and enter or search for the Provider's name.
