This article covers how Clinicians or Administrators can make edits to the list of medications in the Discharge Reconciliation view in Profile. 

## Feature Requirements

- Profile version 10.0.8 or higher
- MedChart should be enabled by adding the Medications licensed module.
- The medications have been [loaded from the MedChart view into the Discharge Reconciliations view](docId\:WeTqGnlcPXVu8uzxYKVoi).

# Edit Discharge Reconciliation Medications List

To access and edit the **Discharge Reconciliation** view:

1. Open up the Medical Record for the applicable patient.
2. Select **MedChart **on the left-hand side. The MedChart view is selected by default. 
3. Select the **Discharge Reconciliation** view\.A number of changes can be made to the medications listed in the **Discharge Reconciliation **view in Profile. These changes can be selected from the toolbar in the view or from the context menu in the list.

## Add

You can add new medications from the formulary or add ad hoc medications (with or without a template) to the list of medications in the same way that you would [add medications in MedChart](docId\:l7lXSS8mxmF9dPIoxx_5J).

## Change 

You can change/edit selected items in the list in the same way that you would in MedChart.

Selecting **Change** allows you to edit the instruction for the medication.

## Delete

You can delete selected items in the list of discharge medications.

**Note: **Updates in the **MedChart **view do **not **auto-sync with the **Discharge Reconciliation **view. This means that new medications added, deleted, or edited in the MedChart view after the Discharge Reconciliation workflow has been triggered are **not **synced. Any changes need to made manually made to the **Discharge Reconciliation **view.
