# Discharge Reconciliation in Profile

Discharge Reconciliation is a feature in Profile that supports safe, efficient, and compliant medication management when patients are discharged from an in-patient setting, such as a hospice or hospital. With Profile’s integrated workflow, providers can reconcile prescribed medications, document decisions, and transfer required medications into the patient’s Usual Scripts with full clinical visibility.

This feature is focused in a dedicated **Discharge Reconciliation **view, which is available with the Medications module.

# Requirements

- The Discharge Reconciliation feature is available in Profile version 10.0.8 and above.
- MedChart should be enabled by adding the Medications licensed module.
