This article is intended for any user wanting to know how to create and fill out forms for a patient using HCare.

# Information

Forms can be filled out from a template created by your organisation and saved as part of a patient encounter.

## Assigning a form to a patient encounter

1. Create a new encounter for the patient. More details can be found on [Add a New Encounter](docId:0Ai6zrWE-LKFzZ1cT1n5Y).
2. On the New Encounter screen, tap **+ Add Form **to bring up a list of Form templates in the system.

3. Tap the applicable Form template
4. Fill out the required details listed in the Form
5. Tap **Save** on the top-right corner, and choose either **Save **or **Save as Incomplete**. 

If a form is saved as **Save as Incomplete**, it will be found under the **Incomplete Items** option of Provider Home
