# Add Signature Field to a CDO Form (Form Templates)

### Pre-requisites

1. The provider must have a Digital Signature image stored on their User account > Special Tab > Digital Signature field. These steps are outlined in the article [Creating a Digital Signature for a Provider.

### Configure the Digital Signature to populate into a Form Template

Open the Form Templates window

1. Maintain Menu
2. Templates
3. Forms

Locate and Edit the form

1. Right-click the form
2. Select Modify

Click the “Open Form Editor” button to edit the form template.

Add a new control

1. In the Control Palette, click the “Rich Text Editor” button.

Add the new control by clicking an empty space on the form

Configure the control to populate a signature

1. Select your Control
2. In the Object Inspector window, next to “Populate Value”, click the **\[…] **button.

Insert a variable for populating a Providers Signature by clicking the** \[…] **button and browsing for variables. Some choices are:

1. «Letter.LoggedOnUser.Signature»  This variable uses the signature of the person who opened the form.
2. «Letter.Patient.CareTeam("Usual Provider").Signature»  This variable uses the signature of the provider assigned to **Usual Provider** on the alter patient window.

Close all form editing windows, and click “Yes” to save the form.
