add signature field to a cdo form (form templates) pre requisites the provider must have a digital signature image stored on their user account > special tab > digital signature field these steps are outlined in the article creating a digital signature for a provider docid\ iipmgadjesq tmek2q4cn configure the digital signature to populate into a form template open the form templates window maintain menu templates forms locate and edit the form right click the form select modify click the “open form editor” button to edit the form template add a new control in the control palette, click the “rich text editor” button add the new control by clicking an empty space on the form configure the control to populate a signature select your control in the object inspector window, next to “populate value”, click the \[…] button insert a variable for populating a providers signature by clicking the \[…] button and browsing for variables some choices are «letter loggedonuser signature» this variable uses the signature of the person who opened the form «letter patient careteam("usual provider") signature» this variable uses the signature of the provider assigned to usual provider on the alter patient window close all form editing windows, and click “yes” to save the form
