# Problem

The print-out pathology and radiology requisition forms don't have the data of clinical details.

# Solution

Steps taken:

1. Organization
2. External Provider
3. Select "Suppliers" (Type) and "Pathology" or "Radiology" (Specialty)
4. Order Format
5. Select "BC Investigation Form" (ensure not "BC investigation Form (3 column)")
