Profile offers a comprehensive system of managing patients with chronic diseases. You can create, view and manage CDM forms and view Flow Sheets. ## CDM Form Creation from a New Encounter  1. Double-click a form from the **Most Used Form Templates** area or press **Browse **to search for a new form. 3. Complete any required fields. 5. If the patient is to be deregistered from the toolkit, check the **Toolkit deregister reason **box and select a reason. 6. When you are finished, select **Complete** from the **Form Status*** *dropdown menu. 10. Press** Save **to save the form. Press **Yes**  in the dialog box if you want to add the diagnosis to the encounter and add the form to the current encounter note. 11. Select the name of the flow sheet you want view from the **Flow Sheets** list. The table will display the guidelines, the last value entered and the most recent date these values were entered. A cumulative table of values is displayed in date columns.
