this guide reviews how to create, view and manage pito cdm forms profile offers a comprehensive system of managing patients with chronic diseases you can create, view and manage cdm forms and view flow sheets cdm form creation from a new encounter select the form view in the actions segment bar menu double click a form from the most used form templates area or press browse to search for a new form double click on the form you want to create this opens a new cdm form for that disease the general tab allows you to enter new measurements and shows you the last measures entered and whether they are outdated according to the bcma guidelines this is symbolized with a beside the date of the value note labs can be ordered directly from this area by checking the check boxes in the examinations panel complete any required fields if the patient is to be deregistered from the toolkit, check the toolkit deregister reason box and select a reason press next to move to the next tab the \[disease] tab allows you to enter disease specific information you can access various information sheets by clicking the buttons on the right hand side complete any necessary information press next to move to the next tab the \[disease] notes tab allows you to view the last 12 months of encounters related to the disease when you are finished, select complete from the form status dropdown menu press save to save the form press yes in the dialog box if you want to add the diagnosis to the encounter and add the form to the current encounter note to print the labs you have ordered through the cdm form, go to the pathology view on the actions segment bar menu and click cdm forms access from the medical record select the forms view from the medical record you will see a list of forms on the left hand side select the form you want to view and it will display to the right of forms type list note in the status c olumn of the forms type list, parked indicates the form is incomplete flow sheets (8 5 and before) select flow sheets from the segment bar menu of the medical record select the name of the flow sheet you want view from the flow sheets list the table will display the guidelines, the last value entered and the most recent date these values were entered a cumulative table of values is displayed in date columns use the following icons on the toolbar to perform the following actions to show measures in a graph format to show measures in a tabular format to create a manual transaction to add prior medications to the flowsheet attachments
