overview & requirements this article is a reference for patients using profile's online booking solution it will cover the online booking appointment process that they will experience requirements profile versions 9 0 and above online booking appointment process for patients getting started click on or enter the url in your browser click the make online booking button, to get started patient verification the patient will enter in the following credentials first name last name health card number the patient's personal health number (phn), used in profile date of birth click next select the place of service (pos) & provider select the pos for the appointment select from the list of providers displayed this will be based on the clinic selected click the next button provide the appointment reason select the appropriate engagement type options include in person phone appointment video appointment select the appointment type from the drop down list enter a reason for the appointment if the engagement type is a phone appointment , the phone number that the patient wishes to be contacted at must be entered in the phone number must contain between 10 and 21 digits click the next button select time and date select the necessary month and day select the available timeslots these are based on the availability of the provider previously selected click the next button confirmation & summary this is the final confirmation screen where all of the selected appointment details will be displayed if this is a phone appointment, the patient must click the acknowledgement checkbox to say that they acknowledge that the appointment will not take place at the clinic click the book now button to finalise and book the appointment a secondary confirmation will display select confirm an appointment will be created for the patient with the selected clinic and provider booking confirmation screen once the appointment has been confirmed, a final confirmation screen will appear this screen will include a summary of the appointment booked users can easily print a copy of the booking information for ease of reference push the print button to print the confirmation page to return to the homepage, click the go to homepage button
