# Antenatal Form

The Antenatal form in Profile allows users to chart and track pregnancy related information for patients.

The form was resdesigned in late 2023 to better align with the paper form. 

# Workflow

## New Pregnancy Creation

1. Go to **Medical Record/Obstetric**.

2. In the **Pregnancies** toolbar, click

3. Fill out the date in the **LMP** and/or **Scan Date** fields as appropriate. If a positive pregnancy test occurred, enter the date in the **Pos Pregnancy Test** field and select the appropriate radio button in the **Certain** area.
4. Enter the **Nature of Pregnancy** and contraceptives information in the **Background** panel.
5. Press

The Pregnancy window can also be used to enter past pregnancies.

## Creation of a New Form

Our recommendation is to initiate a new antenatal form from the medical record rather than a New Encounter because the**Obstetric** view allows you to check if pregnancies already have an associated form.

1. Go to **Medical Record/Obstetric**.

2. As the newest pregnancy is selected by default, click

3. In section** 1** of the form, patient information is automatically populated from **Alter Patient** and additional information, such as Primary Maternity Care Provider, may be recorded here.Section** 2Allergies** populates with patient adverse reaction information from **Clinical Details/Adverse Reactions,** and you can enter folic acid and beliefs/practices in this section.Section** 3** will automatically populate information from the pregnancy linked to the antenatal form. Section **4Obstetrical History** populates previous pregnancies recorded in the patient **Medical Record/Obstetric** and additional data may be entered in the corresponding columns located under **Obstetrical History.**
4. Press

### Part 1 

In the **Part 1** tab, complete the necessary fields and checkboxes (certain fields are unavailable until a corresponding checkbox is selected). Press

in section** 10 Initial Physical Examination** to populate the most recent values into the measure fields.

### Part 2 

1. Select the **Part 2** tab and complete the necessary fields and checkboxes.

### Additional Antenatal Form Tabs

The** Discussion Topics** tab can be used to track antenatal related topics discussed or to be discussed with your patient with checkboxes.

The** Additional Information** tab has a rich text enabled field where any additional information may be recorded.

The **Reference** tab contains more detailed information related to specific sections of the Antenatal Form. Clicking the section** 17 Prenatal Visits Notes** graph icon will display a fundal height graph with fundus measurements.

The **Clinical Notes** tab displays notes and recorded observations from **Antenatal Observations**,** Antenatal Form**, and pregnancy-coded **Encounters** in a consolidated format. The same notes can also be found within the patient’s **Encounters **view of their medical record.

Only Encounter notes with the same Pregnancy code (each pregnancy is assigned a number) will be displayed together.

## Antenatal Observations for Subsequent Visits

1. Go to **Medical Record/Obstetric**.
2. In the **Antenatal Obs **toolbar, click

## Pregnancy Completion

1. Go to **Medical Record/Obstetric**.
2. In the **Pregnancies** toolbar, click

3. Complete the required **Labour** fields. 
4. Select the **Infants** tab and double-click the infant line to edit the information.
