This article is intended for users wanting to know how to create a Problem for a patient using hCare.

# Information

1. From the Provider Home, tap the profile of the patient to enter Patient Home.
2. Tap the (+) button on the bottom-right corner and choose **New Problem for \[Patient]**

3. Tap the **Type **field to choose the applicable problem type.
4. If needed, tap the date field to change the date.
5. If needed, tap the **Case **field to assign the Problem to a specific patient's Case.
6. In the **Code **field, type in the appropriate Service Code and select it from the dropdown list. The **Description **field will be filled automatically
7. If the Type chosen was **Adverse**, you will also need to fill out the **Nature **field with the nature of the adverse reaction, as well as the severity of the adverse reaction in the **Severity **field. The **Nature Description** field will be filled automatically.

8. Tap **Save **in the top-right corner to save the Problem.

Problems can be found on the **Problems **menu on the left-hand side in the Patient Home screen.

You can tap on an existing problem to edit it.

Once a Problem has been added via this method, the Problem can be included as part of a New Encounter. See [Add a New Encounter](docId:0Ai6zrWE-LKFzZ1cT1n5Y) for more details.
