To add coded problems, complete the following steps:

Open the Clinical Details window for the patient. IS IT VIEW OR WINDOW?

Click the **Problems** tab.

Click **Add**.

- The Problem screen appears.

*Note: If you click the arrow beside the **Add** icon, you can select Diagnosis, Procedure or Social/Risk as specific problem type.*

Enter a code by typing the ICD-9 number or part of the name.

*Note: If you enter part of the code name, a list of possible matches appears. Select the appropriate code from the list.*
