# Overview

Patient Accession allows the patient to be an active participant in their care.   Depending on the access permissions set up by the practice, the patient can make appointments, request prescription refills and review their clinical information, among many other actions. This document describes all areas of the Patient Accession web portal. This article is targeted at users of Accession.

# Details

## Logging in to Accession

When Accession is licensed, your clinic will be given a URL to provider your patients with to use the Accession web portal. Access to the web portal is granted on a patient-by-patient basis, and is configured within Profile. More information on granting permission to the web portal can be found in the Accession and Health Connect Configuration documentation.

When the patient navigates to the URL, they will be prompted to enter their username and password. After successful login, they will be in the web portal at the Home view.

## Home

The Home view shows Headline information and any created Alerts for patients.  The Alerts are configured as mentioned in Health Connect Configuration.

## Appointments

The Appointments view, accessible from the top toolbar, shows past and pending appointments.

To book an appointment:

1. From the Appointments view, press

2. Select a date range in the Look Between fields, select the appropriate POS, provider, and appointment type, and press

## New Clinical Note

The New Clinical Not&#x65;**&#xA0;**&#x76;iew allows a patient to enter his or her own clinical notes and/or observations.

Patient-created clinical notes appear in the Work Centre/Accession Items view to be reviewed by the provider. The provider can sign the note, which will then appear as an Encounter in the patient’s medical record, with the author being the patient.

There are two panels in this view. The Clinical Note panel is a free text area where notes can be typed. The bottom Clinical Observation&#x73;*&#xA0;*&#x70;anel can be used utilized for custom forms. These are the same CDO forms as found in or created\* in Maintain/Templates/ Health Record Collections. They are linked to Accession in the Organisation/ Preferences/Profile Patient Portal/Templates tab. More information on templates can be found in documentation for Health Connect Configuration.

## Ask a Question

The Ask a Questio&#x6E;**&#xA0;**&#x76;iew allows the patient to ask the provider a question.

Patients can attach files to their questions, such as picture. Questions appear in Work Centre/Accession Items view, to be answered by the physician.

## Problem List

The Problem List view shows the current problems for the patient, including diagnoses, adverse problems and procedures.

Select **?** at the end of each problem to access a summary and web link to more information.

By default these are blank and must be customized by the practice. These are configured by going t&#x6F;**&#xA0;Organisation/Preferences/Profile Patient Portal/Text&#xA0;**&#x74;ab.  Select the Problem List/Info option. This opens a new window, where a list of coded problems can be created for which you wish to add additional information.

Practices using Accession can add instruction information to a disease for patients to review:

In order to display these instructions, a template must first be configured:

1. Navigate to Maintain/Templates/Instructions. The Instruction Templates window will open.
2. Press the

3. In the Title field, enter a description for the template, such as the name of the disease.
4. In the Disease field, select a Termset from the drop-down menu (ICD-9-CM is most common) and enter the associated disease code in the field to the right.
5. In the Measure field, select a Termset from the drop-down menu (intraHealth is most common) and enter the associated measure item in the field to the right.

6. Press OK to save the new Instruction Template.
   To associate a template with a disease, it must then be added via Clinical Details:
7. Open Clinical Details for a specific patient.
8. Under the Problems tab, double-click on the applicable Problem to open it for editing.
9. Under the Instructions tab, press the

10. In the Template field, enter the name of a template created previously.

The instructions will now be visible for the specific patient when they view their Problem List through Accession.

To be able to configure Instruction Templates, the Role Action Maintain/Templates/Instructions must be granted for the user/role configuring the Instruction Templates.

## Care Plans

The Care Plan&#x73;**&#xA0;**&#x76;iew shows interventions that are still due or all interventions, depending on which radio button is selected.

Appointments for the Interventions can be made from here using the Make Appointment button. These appointments will automatically be linked to the associated intervention in Profile, provided the Interventions have been added as part of an Appointment Rule.

## Prescriptions

The Prescriptions view shows current prescriptions and allows the patient to ask for prescription refills, pending approval by the provider.

This is the one interaction that does not appear in the Work Centre/Accession Items view. The information will show directly in the Work Centre/Prescription Refills view. The Prescription Refills module may not be enabled for your practice. Further documentation on Prescription Refills is available on the Intrahealth Self Service Centre website.

## Investigations

The Investigations view is similar to the Result&#x73;**&#xA0;**&#x76;iew in Profile, lab results and radiology results can be seen from here.

## Referrals

Th&#x65;**&#xA0;**&#x52;eferral&#x73;**&#xA0;**&#x76;iew is similar to the Referrals view in Profile. Current and past inward or outward referrals can be seen from here.

## Flow Sheets

The Flow Sheet&#x73;**&#xA0;**&#x76;iew is similar to the Flow Sheet&#x73;**&#xA0;**&#x76;iew in Profile. Chronic disease observations from existing CDM forms are tabulated for comparison.

## Clinical Outcomes

The Clinical Outcomes view is similar to the Measures view in Profile. Measures are displayed based on date range and can be displayed as either a Graph or Table, using the Show As dropdown.

## Clinical Notes

The Clinical Note&#x73;**&#xA0;**&#x76;iew is similar to Encounter&#x73;**&#xA0;**&#x76;ie&#x77;**&#xA0;**&#x69;n Profile. Clinical notes are shown and can be sorted by their associated problems.

## Reference Series

The Reference Serie&#x73;**&#xA0;**&#x76;iew is similar to the New Encounter/Reference Serie&#x73;**&#xA0;**&#x76;ie&#x77;**&#xA0;**&#x69;n Profile. For patients with existing reference series/growth chart data, the appropriate reference series can be selected from the drop-down menu and displayed. 

The Show values checkbox will enable labelling the measure values directly onto the graph.

## Linked Clinical Records

The Linked Clinical Record&#x73;**&#xA0;**&#x76;iew allows patients to add links to other patients’ Accession portals, such as their family members.

To link other Accession-enabled patients:

1. Press the Add link button to add another patient.
2. Enter the User name and Password for that patient’s Accession portal.

3. The patient will now be visible in the My linked clinical records section.
4. To open another patient’s Accession portal, simply click on their name.

5. Patients who have had their clinical record linked through another patient’s Accession portal will see the names of patients who they have been linked with in the My record is linked by section.

## Account

The Account view is similar to Display Transaction&#x73;**&#xA0;**&#x69;n Profile. Financial transaction history for the patient is shown here.

If PayPal integration is required for your practice, please contact Intrahealth to have a statement of work created.

## Change Your Details

The Change Your Detail&#x73;**&#xA0;**&#x76;iew is similar to Alter Patien&#x74;**&#xA0;**&#x69;n Profile. From here, patients may change demographic details, such as updating their address or phone numbers.

Additionally, the patient may manage their Security Questions in the event they are unable to log into Accession.

Changes made appear in Work Centre/Patient Items view. The changes made by the patient appear in red and are automatically updated in Profile. The clinic can accept the changes, or roll back the changes, reverting to the original information.

## Log Off

The Log Of&#x66;**&#xA0;**&#x76;iew allows the patient to log off of Accession.
